Spine Points
Within each band, there are spine points — annual increments you move up:
- Usually progress 1 point per year (on your anniversary)
- Some bands have 2-9 spine points
- Promotions move you to a new band (usually lowest point)
- Can be accelerated for excellent performance (Trust discretion)
NHS Deductions Explained
1. Income Tax
England, Wales, Northern Ireland:
- Personal Allowance: £12,570 (tax-free)
- Basic rate (20%): £12,571 - £50,270
- Higher rate (40%): £50,271 - £125,140
- Additional rate (45%): £125,141+
Scotland:
- Personal Allowance: £12,570
- Starter rate (19%): £12,571 - £15,397

2. National Insurance (NI)

3. NHS Pension
Contribution rates based on pensionable pay (2026/27):
- Up to £13,259: 5.2%
- £13,260 - £26,831: 6.5%
- £26,832 - £32,691: 8.3%
- £32,692 - £50,060: 9.8%
- £50,061 - £63,210: 10.7%
- £63,211 - £74,225: 11.6%

4. Student Loan Repayments
- Plan 1: 9% above £24,990/year
- Plan 2: 9% above £27,295/year
- Plan 4 (Scotland): 9% above £31,395/year
- Postgraduate: 6% above £21,000/year

London Weighting (HCAS)
High Cost Area Supplements for London-based staff:
- Inner London: 20% of basic (£6,009 - £7,297/year)
- Outer London: 15% of basic (£4,507 - £5,472/year)
- Fringe: 5% of basic (£1,502 - £1,824/year)
Scotland vs England
Pay scales:
- Scotland has separate pay scales (often slightly higher)
- Different negotiation processes

Tax:
- Scotland has 6 tax bands (vs 3 in England)
- Scottish taxpayers typically pay more tax on £30k-50k salaries
