NHS Agenda for Change Pay Scales 2026/27 — England
The following pay scales apply to all NHS Agenda for Change staff in England, Wales, and Northern Ireland from 1 April 2026. These figures include the 3.3% pay rise agreed for 2026/27.
Annual salary versus “money in hand” narratives
Policy headlines quote salary numbers; unions quote percentage uplifts adjusted for CPI/RPI inflation. Workforce finance teams reconcile both but staff mainly feel cashflow. That is why pairing gross spinal tables — what you negotiate — with illustrative net deductions matters: it prevents misconceptions like “£1,000 extra gross means £1,000 extra spending money,” which overlooks higher pension tiers and widening basic-rate bands.
When budgeting, simulate three horizons: baseline net after core deductions, scenario with postgraduate loan deductions, scenario with childcare salary sacrifice diminishing pensionable pay. Our tools separate those toggles deliberately.

How AfC Pay Bands and Spine Points Work
Pay Bands
NHS Agenda for Change uses 9 pay bands (Bands 2-9) organized by role responsibility and skills required:
- Bands 2-4: Support and assistant roles
- Bands 5-7: Registered professionals and specialists
- Bands 8a-9: Advanced practitioners, consultants, and managers
Spine Points (Annual Increments)
Within each band, there are spine points — step increases in pay:
- You move up one spine point each year on your anniversary date
- Each band has 2-9 spine points
- When you reach the top of your band, you stop progressing (unless promoted)
- Increments are automatic (not performance-based in most cases)
Example: Band 5 Nurse Career Progression
Year 1 (2026): Point 1 — £30,049
Year 2 (2027): Point 2 — £31,049 (+£1,000)
