How NHS Overtime Pay Works
NHS Agenda for Change staff receive enhanced pay rates for working unsocial hours, weekends, nights, and bank holidays. These enhancements are paid on top of your basic hourly rate.

How to Calculate Your Overtime Rate
Step 1: Calculate your basic hourly rate
Annual Salary ÷ 52 weeks ÷ Contract Hours = Hourly Rate
Example: £32,073 ÷ 52 ÷ 37.5 = £16.45/hour
Step 2: Apply the enhancement multiplier
- Night shift: £16.45 × 1.30 = £21.39/hour
- Saturday: £16.45 × 1.30 = £21.39/hour
- Sunday: £16.45 × 1.60 = £26.32/hour
- Bank Holiday: £16.45 × 1.60 = £26.32/hour
Step 3: Multiply by hours worked
8 hours Sunday × £26.32 = £210.56 gross
What Counts as Overtime?
Overtime is any hours worked beyond your contracted hours. For example:
- Full-time (37.5 hours/week) staff working extra shifts
- Part-time staff working above their contracted hours
- Additional hours during staff shortages
- On-call work when called in
Bank Holiday Enhancements
8 UK Bank Holidays per year:
- New Year's Day (1 January)
- Good Friday
- Easter Monday
- Early May Bank Holiday
- Spring Bank Holiday (late May)
- Summer Bank Holiday (late August)
- Christmas Day (25 December)
- Boxing Day (26 December)
Working any of these days = 60% enhancement on all hours worked.
Night Shift Rules
Night shift enhancement (30%) applies to any hours worked between 8pm and 6am, regardless of day. This means:
- A night shift on Saturday 8pm-8am gets both Saturday (30%) AND night (30%) enhancements
- Sunday night into Monday gets Sunday enhancement (60%) until midnight, then night enhancement (30%)
- Bank holiday nights get bank holiday (60%) + night (30%) enhancements
How Overtime is Paid
Payment timing:
- Overtime usually paid in the following month's payslip
- Submit timesheets by your Trust's deadline (usually 5th-10th of month)
- Late submissions may be paid in month after next
Tax and deductions:
- Overtime is taxed as normal income (20%, 40%, or 45% depending on your total earnings)
- National Insurance applies (8% up to £50,270, then 2%)
- NHS Pension contributions apply (your normal pension tier percentage)
- Student loan repayments apply if you have a loan
Rough take home: Expect around 60-70% of gross overtime after all deductions.
Maximum Weekly Hours
Under the Working Time Directive:
- Maximum 48 hours per week averaged over 17 weeks
- You can opt out of this limit (check with your Trust)
- 11 hours rest between shifts (can be averaged)
- 24 hours uninterrupted rest per week (or 48 hours per fortnight)

Example 1: Band 5 Nurse — Sunday Shift
- Annual Salary: £32,073
- Hourly Rate: £16.45
- Sunday Enhancement: £16.45 × 1.60 = £26.32/hour
- 12-hour Sunday shift: £26.32 × 12 = £315.84 gross
- Take home (approx): £205
Example 2: Band 6 — Bank Holiday Night Shift
- Annual Salary: £39,959
- Hourly Rate: £20.49
- Bank Holiday Night: £20.49 × 1.60 × 1.30 = £42.62/hour (stacking enhancements)
- 8-hour shift: £42.62 × 8 = £340.96 gross
- Take home (approx): £220
